Why would a disease go through all the effort?

Everyone knows about terminal diseases. They strike hard, fast, without warning and you're probably gone within a few hours maximum. I know that this is quite a sensitive subject (I have no intention in offending anyone) but why? Why would a diseases go through all the effort just to kill the host and then end up killing itself in the process?
